You should verify the version of software within the pack. Upon reset of the unit, the software version will display. This version should be at least 1.0.7. If it is running an earlier version of code, this may the problem. You should get the serial number of the pack and call ETC Service at 1-800-688-4116 and arrange to have a configured control board sent out at the earliest convenience.
If you are running 1.0.7, reset the pack and test the loads by running all of the dimmers up from the pack.
The problem could be a bad power cube or control card. The power cube can be changed on-site, but some users may find it difficult. To remove, pull the cube straight up from the power board. The cube is connected with fast-on connectors and straight pins to the power board. There is a certain amount of force required to disconnect the cube from the board. When the user replaces the cube, it is crucial that the pins be correctly aligned before connecting to the power board.
